With the changes coming in health care, I would say sooner is better than later. I've heard of deductibles going from $1000 to $15000 plus it's an elective surgery. It's not going to get better with shots and meds, you just won't feel the pain as much but it's still there. I've rehabed hundreds of pts through these and I can't tell you how many have said they wished they had done it sooner. After the surgery, control the swelling and you control the knee.
