Not sure on the reflective law or how big an area they want but on some helmets the dot approval on the back is reflective, or to comply and be a bit safer one could buy small reflective stickers.
As for the railway crossings I think what Tom may have meant was 45 degree as a minimum. We have rural railway crossings here in Quebec and Ontario that intersect roads at a angle sometimes making it hard to cross the tracks at a true 90 degrees.
